Abstract: The article aims to analyze categories of the exercise of power within the political and legislative processes related to proposals to amend Law No. 11.771/2008, known as the General Tourism Law and the main legal framework of Brazilian tourism. Through a qualitative approach, we analyzed bills that alter the norm, as well as drafts and expressed justifications. Communication notes from tourism organizations and minutes of meetings of the National Tourism Council were also analyzed. The study revealed that the main parties involved with the changes are the public sector, members of the National Tourism Council, and private groups. The changes focus on favoring private business in tourism, and although the National Tourism Council is not the only forum for presenting the interests of those involved, it has proven to be a relevant space for joint reflection on changes in the norm. The research highlights the need to identify those involved and their interests in relation to a norm that is structuring Brazilian tourism. Moreover, it shows that tourism is an area in constant dispute with agents and organizations that exercise their power and this entails consequences for tourism and its public policies.

Resumen: El artículo tiene como objetivo analizar las categorías del ejercicio del poder dentro de los procesos políticos y legislativos relacionados con las propuestas de modificación de la Ley nº 11.771/2008, conocida como Ley General de Turismo y principal marco jurídico del turismo brasileño. A través de un enfoque cualitativo, se analizaron los proyectos de ley que modifican la norma, así como los borradores y las justificaciones expresadas. También se analizaron las notas de comunicación de las organizaciones turísticas y las actas de las reuniones del Consejo Nacional de Turismo. El estudio reveló que los principales implicados en los cambios son el sector público, los miembros del Consejo Nacional de Turismo y los grupos privados. Las modificaciones se enfocan en favorecer el negocio privado en el turismo y, aunque el Consejo Nacional de Turismo no es el único medio para presentar los intereses de los involucrados, demostró ser un espacio relevante para la reflexión conjunta sobre los cambios en la norma. La investigación destaca la necesidad de identificar a los involucrados y sus intereses en relación a una norma que está estructurando el turismo brasileño. Además, muestra que el turismo es un área en constante disputa con agentes y organizaciones que ejercen su poder y esto acarrea consecuencias para el turismo y sus políticas públicas.

Resumo: O artigo tem o objetivo de analisar categorias do exercício do poder no âmbito dos processos políticos e legislativos relacionados às proposições de alteração da Lei n. 11.771/2008, conhecida como Lei Geral do Turismo e principal marco legal do turismo brasileiro. Por meio de uma abordagem qualitativa, foram analisados projetos de lei que alteram a norma, além de minutas e justificativas expressas. Também foram averiguadas notas de comunicação de organizações do turismo e atas de reuniões do Conselho Nacional de Turismo. O estudo revelou que os principais envolvidos com as alterações são o setor público, integrantes do Conselho Nacional de Turismo e grupos privados. As alterações têm como foco o favorecimento dos negócios privados em turismo e, embora o Conselho Nacional de Turismo não seja o único meio para apresentação de interesses dos envolvidos, ele demonstrou ser um espaço relevante para reflexão conjunta sobre alterações na norma. A investigação destaca a necessidade de identificar envolvidos e seus interesses com relação a uma norma estruturante do turismo brasileiro. Ademais, evidencia que o turismo é uma área em constante disputa com agentes e organizações que exercem seu poder e isso acarreta consequências para o turismo e suas políticas públicas.

ABSTRACT OBJECTIVE To analyze the association between municipal rates of ambulatory care sensitive conditions (ACSC) hospitalization and the quality of primary health care (PHC), socioeconomic, and demographic variables and those related to local characteristics of the health system from 2010 to 2019. METHOD Ecological time series study in Brazilian municipalities analyzing the correlation of ACSC hospitalization rates with PHC quality measured by the three cycles of the Primary Care Access and Program for improving primary care access and quality (PMAQ-AB). The study included municipalities whose teams participated in 80% or more of at least two PMAQ-AB cycles. The correlation between standardized ACSC hospitalization rates and PHC quality and other variables was analyzed. Spearman’s test was used between the response variable and numerical explanatory variables. Generalized equations estimation was used as a multivariate model associating ACSC hospitalization rates with the other variables over the years. RESULTS A total of 3,500 municipalities were included in the models. The quality of PHC (PMAQ-AB score) showed an inverse association with the variation in ACSC hospitalization rates. Hospitalization rates fell by -2% per year every ten-point increase in the PMAQ-AB score, adjusted by the remaining variables. A one-unit increase in the beds per 1,000 inhabitants variable had an impact of approximately +6.4% on ACSC hospitalization rates. Regarding population size, larger municipalities had lower ACSC hospitalization rates. Increased PHC coverage and lower socioeconomic inequality were also associated with the reduction in hospitalizations. CONCLUSIONS The reduction in ACSC hospitalization rates over time was associated with an increase in the quality of PHC. It was also associated with a reduction in the number of hospital beds and municipalities with better socioeconomic indicators.

RESUMO OBJETIVO Analisar a associação entre taxas municipais de internações por condições sensíveis à atenção primária (ICSAP), com a qualidade da atenção primária à saúde (APS), variáveis socioeconômicas, demográficas e relacionadas a características locais do sistema de saúde, de 2010 a 2019. MÉTODOS Estudo ecológico de séries temporais nos municípios brasileiros analisando a correlação das taxas de ICSAP, com a qualidade da APS medida pelos três ciclos do Programa de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB). Foram incluídos municípios que participaram com 80% ou mais de suas equipes em, ao menos, dois ciclos do PMAQ-AB. Foi analisada a correlação entre as taxas de ICSAP padronizadas com a qualidade da APS e demais variáveis. Empregou-se o teste de Spearman entre a variável resposta e as variáveis explicativas numéricas. Foi usado o generalized equations estimating como modelo multivariado associando as taxas de ICSAP e as demais variáveis ao longo dos anos. RESULTADOS Foram incluídos 3.500 municípios nos modelos. A qualidade da APS (nota do PMAQ-AB) apresentou associação inversa com a variação das taxas de ICSAP. As taxas de internação tiveram queda de -2% ao ano a cada aumento de dez pontos na nota do PMAQ-AB, ajustado pelas demais variáveis. O aumento de uma unidade na variável leitos por mil habitantes impactou em uma elevação de aproximadamente +6,4% nas taxas de ICSAP. Quanto ao porte populacional, municípios maiores tiveram menores taxas de ICSAP. Também se associaram à redução das internações o aumento da cobertura da APS e a menor desigualdade socioeconômica. CONCLUSÕES A redução das taxas de ICSAP ao longo do tempo mostrou-se associada com o aumento da qualidade da APS. Além disso, esteve associada com diminuição do número de leitos hospitalares e a municípios com melhores indicadores socioeconômicos.

Abstract Background Literature is scarce on echocardiographic characteristics of COVID-19 patients admitted to the intensive care unit (ICU). Objectives To describe echocardiographic characteristics of ICU COVID-19 patients and associate them with clinical signals/symptoms, laboratory findings and outcomes. Methods Patients with RT-PCR-confirmed COVID-19, admitted to the ICU, who underwent echocardiography were included. Clinical characteristics associated with an abnormal echocardiogram (systolic ventricular dysfunction of any degree — left and/or right ventricle — and/or high filling pressures and/or moderate to severe pericardial effusion) were analyzed. Groups were compared using the Student’s t-test, chi-square, and logistic regression. A p < 0.05 was considered statistically significant. Results A total of 140 patients met inclusion criteria, and 74 (52.9%) had an abnormal echocardiogram. A low number of left and right ventricular systolic dysfunction was observed, and 35% of the population had a normal diastolic function. In the univariate analysis, characteristics associated with abnormal echocardiogram were age, chronic kidney disease, elevated troponin, previous heart failure, and simplified acute physiology score 3 (SAPS 3). In the regression model, troponin and SAPS3 score were independent markers of abnormal echocardiogram. An abnormal echocardiogram was associated with a higher prevalence of in-hospital death (RR 2.10; 95% CI 1.04-4.24) and orotracheal intubation (RR 2.3; 95% CI 1.14-4.78). Conclusions COVID-19 has little effect on ventricular function, but it is common to find increased filling pressures. Elevated serum troponin level and SAPS3 score were the independent markers of an abnormal echocardiogram. In addition, the prevalence of in-hospital death and need for mechanical ventilation were higher in patients with abnormal echocardiogram.

Abstract: Introduction: The National Curriculum Guidelines (DCN, Diretrizes Curriculares Nacionais) for the undergraduate medical course emphasize the educational training in the context of primary health care and have as axes of the curricular development, the health needs of the population and the teaching, service, and community integration (TSCI), preferably in services of the Brazilian Health System (SUS). Thus, the primary health network is a unique learning scenario, as it provides students the opportunity to experience health policies, as well as the multi-professional work, in addition to the possibility of dealing with real-life problems, associating medical-academic training to the social health needs. Objectives: This work aims to analyze the characteristics of the teaching-service integration, which contribute to the university, health services and the community. It also aims to analyze the difficulties and challenges through the perception of the students, the users and the health professionals involved in the outpatient Surgery Clinic and in the extension project of UFSJ (Universidade Federal de São João del-Rei) in the health units in Tiradentes-MG. Methods: This work is a qualitative study which used, as data collection instruments, semi-structured interviews with users, students and health professionals, about the perception of Teaching Service-Community Integration (TSCI). The Content Analysis technique was used for the data analysis. Results and Discussion: The data showed that the TSCI allowed improving the quality of work in the health service, qualifying the professionals who were working there, in addition to enhancing personal achievement of the actors in this interaction. It also brought improvements to primary care, allowing an understanding of the organization of the work practice and greater effectiveness of the Basic Health Unit. Furthermore, it was possible to perceive the challenges and impasses of this integration to be overcome, such as: lack of structure and materials, a minor discomfort, but no less important, of patients to be cared by students and confrontation of the routine of local health professionals with the University. Conclusion: The university inserted in the social reality and in the primary care services trains professionals who are more qualified for more prevalent problems while benefiting the population and the local health teams.

Resumo: Introdução: As Diretrizes Curriculares Nacionais (DCN) da graduação em Medicina enfatizam a formação no contexto da atenção básica de saúde e têm como eixos do desenvolvimento curricular as necessidades de saúde da população e a integração ensino-serviço-comunidade (Iesc), preferencialmente nos serviços do Sistema Único de Saúde (SUS). Dessa forma, a rede básica de saúde é um cenário de aprendizagem singular, pois proporciona aos alunos a possibilidade de eles vivenciarem as políticas de saúde e o trabalho multiprofissional, além de permitir que lidem com problemas reais, vinculando a formação médico-acadêmica às necessidades sociais da saúde. Objetivo: Este estudo teve como objetivo analisar as características da integração ensino-serviço que contribuem para a universidade, os serviços de saúde e a comunidade, e as suas dificuldades e os seus desafios por meio da percepção dos discentes, usuários e profissionais de saúde envolvidos no ambulatório de cirurgia ambulatorial e no projeto de extensão da Universidade Federal de São João del-Rei (UFSJ) nas unidades de saúde em Tiradentes, em Minas Gerais. Método: Trata-se de estudo qualitativo que utilizou como instrumentos de coleta de dados entrevistas semiestruturadas com usuários, discentes e profissionais de saúde sobre a percepção da IESC. Para a análise dos dados, empreendeu-se a técnica de análise de conteúdo. Resultado: Os dados mostraram que a IESC permitiu melhorar a qualidade do trabalho no serviço de saúde, qualificar os profissionais ali presentes, além de ampliar a realização pessoal dos atores dessa interação. Também trouxe melhorias à atenção primária, possibilitando a compreensão da organização da prática no trabalho e maior resolubilidade da unidade básica de saúde. Ainda foi possível uma percepção dos desafios e impasses dessa integração a serem superados, como falta de estrutura e materiais, desconforto minoritário, mas, não menos importante, de pacientes a serem atendidos pelos estudantes e confronto da rotina dos profissionais de saúde locais com a universidade. Conclusão: A universidade inserida dentro da realidade social e dos serviços de atenção primária forma profissionais mais capacitados para problemas mais prevalentes ao mesmo tempo que beneficia a população e as equipes de saúde locais.

Abstract Background Short and long-duration heart rate variability (HRV) data from Holter monitoring could identify predictors of all-cause death in heart failure (HF) patients. Objectives To build a predictive model of all-cause death in patients with HF using HRV. Methods Retrospective study including patients with suspected or confirmed HF who were admitted for decompensated HF or syncope that underwent Holter monitoring. In analysis of augmented sympathetic tonus, we evaluated the lowest HRV in nonoverlapping 10-minutes periods throughout 24h continuous electrocardiographic signal recording (short HRV variables). Variables with p<0.01 were included in a multivariate Cox regression model to determine the occurrence of the all-cause death. Variables with statistical significance in Cox regression were chosen to build the predictive model. P<0.05 was considered significant. Results A total of 116 patients were included, mean age of 71.9±16.3 years, 45.7% men, mean follow-up of 2.83±1.27 years. Thirty-nine deaths occurred (33.6%). By comparing survivors vs. non-survivors, the variables that showed statistical significance were lowest SDNN, lowest rMSSD, age and left ventricular ejection fraction (LVEF). In Cox regression, independent predictors of all-cause death were: age>69 years (HR 3.95, 95%CI 1.64-9.52); LVEF≤57% (HR 4.70, 95%CI 2.38-9.28) and lowest rMSSD≤12ms (HR 5.54, 95%CI 2.04-15.08). An integer value was assigned to each variable. Score<3 showed AUC=0.802 (95%CI 0.72-0.87). Conclusion In HF patients hospitalized for decompensated HF or syncope, independent long-term predictors of all-cause death were age, LVEF, and 10-minutes rMSSD. These findings indicate that even brief moments of high sympathetic tone can impact survival, specifically in the elderly and patients with HF with reduced ejection fraction.

Resumo Fundamento Dados de curto e de longo prazo da variabilidade da frequência cardíaca (VFC) poderiam identificar preditores de mortalidade por todas as causas em pacientes com insuficiência cardíaca (IC). Objetivos Construir um modelo preditivo de mortalidade por todas as causas em pacientes com IC usando a VFC. Métodos Estudo retrospectivo incluindo pacientes com suspeita ou diagnóstico confirmado de IC internados por IC descompensada ou síncope e que realizaram exame de Holter 24 horas. Na análise do tônus simpático aumentado, nós avaliamos a VFC mais baixa em períodos de 10 minutos não sobrepostos em um registro contínuo de sinal eletrocardiográfico por 24 horas (VFC de curta duração). As variáveis com p<0,01 foram incluídas no modelo de regressão multivariada de Cox para determinar a ocorrência da mortalidade por todas as causas. As variáveis com significância estatística na regressão de Cox foram escolhidas para construir o modelo preditivo. Um p<0,05 foi considerado estatisticamente significativo. Resultados Um total de 116 pacientes foram incluídos, com idade média de 71,9±16,3 anos, 45,7% eram do sexo masculino. O tempo médio de acompanhamento foi de 2,83 ± 1,27 anos. Trinta e nove (33,6%) óbitos ocorreram. Na comparação de sobreviventes e não sobreviventes, as variáveis que mostraram significância estatística foram menor SDNN, menor rMSSD, idade e fração de ejeção ventricular esquerda (FEVE). Na regressão Cox, os preditores independentes de mortalidade por todas as causas foram: idade > 69 anos (HR 3,95, IC95% 1,64-9,52); FEVE≤57% (HR 4,70, IC95% 2,38-9,28) e menor rMSSD ≤12ms (HR 5,54; IC 95% 2,04-15,08). Um valor inteiro foi atribuído para cada variável. Este escore < 3 apresentou uma área sob a curva de 0,802 (IC95% 0,72-0,87). Conclusão Em pacientes com IC internados por IC descompensada ou síncope, preditores de longo prazo de mortalidade por todas as causas foram idade, FEVE, e rMSSD em 10 minutos. Esses achados indicam que mesmo breves momentos de tônus simpático elevado podem ter impacto na sobrevida, principalmente em idosos e pacientes com IC e fração de ejeção reduzida.

Resumo: O artigo analisa as consequências da Covid-19 para o funcionamento dos conselhos de políticas públicas de turismo dos municípios de Curitiba e Paranaguá durante o ano de 2020 e início de 2021. Baseado na teoria fundamentada, foram analisados dados provenientes de entrevistas com conselheiros e de normas jurídicas dos colegiados. Os resultados identificaram que os conselhos funcionam predominantemente como um fórum de comunicação de ações, que existem dificuldades com relação à responsividade e que a adesão dos conselheiros às reuniões online é caracterizada pela racionalidade limitada. Esse cenário foi intensificado pela Covid-19, principalmente em Paranaguá, enquanto o conselho de Curitiba iniciou ações para enfrentamento da pandemia e aumentou a frequência de reuniões. Conclui-se que as realidades distintas nos casos analisados têm relação com a estrutura do poder executivo de cada município. A experiência dos conselhos durante a Covid-19 é oportuna para repensar o uso da e-democracia, inclusive no contexto pós-pandemia.

Abstract: The article analyzes the consequences of the Covid-19 in the operation of the tourism councils in the cities of Curitiba and Paranaguá, Brazil, during 2020 and early 2021. It was used the grounded theory to evaluate the legal rules that institute the councils and interviews with counselors. The results identified that the councils function predominantly as a forum in order to communicate actions, that there are difficulties in relation to responsiveness and that the participation in the online meetings is characterized by limited rationality. This scenario was intensified by Covid-19, mainly in Paranaguá, while the Curitiba council initiated actions to face the pandemic and increased the frequency of meetings. In conclusion, the different realities are related to the structure of the executive power of each city. The experience of the councils during the pandemics is opportune to consider the use of e-democracy even after the pandemic context.

Resumo Fundamento Alguns estudos demonstraram uma maior prevalência de óbitos em portadores de fatores de risco cardiovascular (FRC) durante internação por COVID-19. Objetivos Avaliar o impacto do alto risco cardiovascular em pacientes internados em terapia intensiva por COVID-19 Métodos Estudo retrospectivo com pacientes admitidos em terapia intensiva, com diagnóstico confirmado de COVID-19 por RT-PCR e com pelo menos uma dosagem de troponina durante a internação. Os critérios para definição de paciente de alto risco cardiovascular (ARC) foram: histórico de doença cardiovascular estabelecida (infarto, AVC ou doença arterial periférica), diabetes, doença renal crônica com clearance < 60ml/min ou presença de 3 FRC (hipertensão, tabagismo, dislipidemia ou idade > 65 anos). O desfecho primário deste estudo é mortalidade hospitalar por todas as causas. P<0,05 foi considerado significativo. Resultados Foram incluídos 236 pacientes, média de idade= 61,14±16,2 anos, com 63,1% homens, 55,5% hipertensos e 33,1% diabéticos. Um total de 47,4% dos pacientes apresentavam ARC. Observou-se um aumento significativo da mortalidade conforme aumento do número de fatores de risco (0 FRC: 5,9%; 1 FRC: 17,5%; 2 FRC: 32,2% e ≥3 FRC: 41,2%; p=0,001). Na regressão logística ajustada para gravidade (escore SAPS3), o grupo de alto risco cardiovascular e troponina elevada apresentou maior ocorrência de mortalidade hospitalar (OR 40,38; IC95% 11,78-138,39). Pacientes sem alto risco cardiovascular, mas com troponina elevada, também exibiram associação significativa com o desfecho primário (OR 16,7; IC95% 4,45-62,74). Conclusão Em pacientes internados em terapia intensiva por COVID-19, a presença de alto risco cardiovascular afeta a mortalidade hospitalar somente em pacientes que apresentaram elevação de troponina.

Abstract Background Some studies have shown a higher prevalence of deaths in patients with cardiovascular risk factors (CRF) during hospitalization for COVID-19. Objectives To assess the impact of high cardiovascular risk in patients hospitalized in intensive care for COVID-19 Methods Retrospective study with patients admitted to an intensive care unit, with a diagnosis of COVID-19 confirmed by RT-PCR, and with at least one troponin measurement during hospitalization. The criteria for defining high cardiovascular risk (HCR) patients were: history of established cardiovascular disease (myocardial infarction, stroke, or peripheral arterial disease), diabetes, chronic kidney disease with clearance < 60ml/min, or presence of 3 CRFs (hypertension, smoking, dyslipidemia, or age > 65 years). The primary outcome of this study is all-cause in-hospital mortality. P<0.05 was considered significant. Results This study included 236 patients, mean age = 61.14±16.2 years, with 63.1% men, 55.5% hypertensive, and 33.1% diabetic; 47.4% of the patients also presented HCR. A significant increase in mortality was observed as the number of risk factors increased (0 FRC: 5.9%; 1 FRC: 17.5%; 2 FRC: 32.2% and ≥3 FRC: 41.2%; p=0.001). In the logistic regression adjusted for severity (SAPS3 score), the HCR and myocardial injury group had a higher occurrence of in-hospital mortality (OR 40.38; 95% CI 11.78-138.39). Patients without HCR but with myocardial injury also exhibited a significant association with the primary outcome (OR 16.7; 95% CI 4.45-62.74). Conclusion In patients hospitalized in intensive care for COVID-19, HCR impacts in-hospital mortality only in patients with myocardial injury.

Resumo Fundamento A importância prognóstica da classificação ‘insuficiência cardíaca (IC) com fração de ejeção (FE) intermediária’ permanece incerta. Objetivo Analisar as características clínicas, comorbidades, complicações e mortalidade hospitalar e tardia de pacientes classificados em IC com FE intermediária (ICFEi - FE: 40%-49%) e comparar às daqueles em IC com FE preservada (ICFEp - FE > 50%) e IC com FE reduzida (ICFEr - FE < 40%) na internação por IC descompensada. Métodos Coorte ambispectiva de pacientes internados por IC descompensada em unidade cardiointensiva. Foram avaliadas características clínicas, comorbidades, complicações e mortalidade hospitalar e tardia. Utilizou-se o software R, com significância de 5%, para a realização dos testes qui-quadrado, análises de variância, multivariada de Cox e curva de sobrevida de Kaplan Meier, além de técnicas de machine learning (Elastic Net, árvore de sobrevida). Resultados Foram incluídos 519 indivíduos entre setembro de 2011 e junho de 2019, com média da idade de 74,87±13,56 anos, sendo 57,6% homens. Observou-se frequência de ICFEp, ICFEi e ICFEr de 25,4%, 27% e 47,6%, respectivamente. O infarto prévio foi mais frequente na ICFEi. O tempo médio de seguimento foi 2,94±2,55 anos, sem diferença estatística da mortalidade entre os grupos (53,8%, 52,1% e 57,9%). Na curva de sobrevida, não houve diferença entre os grupos ICFEp e ICFEi, nem entre ICFEp e ICFEr, mas houve entre os grupos ICFEi e ICFEr. Idade maior que 77 anos, IC prévia, história de readmissão, demência e necessidade de vasopressores foram associadas com maior mortalidade tardia na árvore de sobrevida. Conclusão A FE não foi selecionada como variável associada a mortalidade nos pacientes com IC descompensada.

Abstract Background The prognostic importance of the classification ‘heart failure (HF) with mid-range ejection fraction (EF)’ remains uncertain. Objective To analyze the clinical characteristics, comorbidities, complications, and in-hospital and late mortality of patients classified as having HF with mid-range EF (HFmrEF – EF: 40%-49%), and to compare them to those of patients with HF with preserved EF (HFpEF – EF > 50%) and with HF with reduced EF (HFrEF – EF < 40%) on admission for decompensated HF. Methods Ambispective cohort of patients admitted to the cardiac intensive care unit due to decompensated HF. Clinical characteristics, comorbidities, complications, and in-hospital and late mortality were assessed. The software R was used, with a 5% significance, for the tests chi-square, analysis of variance, Cox multivariate, and Kaplan-Meier survival curve, in addition to machine-learning techniques (Elastic Net and survival tree). Results 519 individuals were included between September 2011 and June 2019 (mean age, 74.87 ± 13.56 years; 57.6% were men). The frequencies of HFpEF, HFmrEF and HFrEF were 25.4%, 27% and 47.6%, respectively. Previous infarction was more frequent in HFmrEF. The mean follow-up time was 2.94 ± 2.55 years, with no statistical difference in mortality between the groups (53.8%, 52.1%, 57.9%). In the survival curve, there was difference between neither the HFpEF and HFmrEF groups, nor the HFpEF and HFrEF groups, but between the HFmrEF and HFrEF groups. Age over 77 years, previous HF, history of readmission, dementia and need for vasopressors were associated with higher late mortality in the survival tree. Conclusion The EF was not selected as a variable associated with mortality in patients with decompensated HF.

Resumo Fundamentos Uma metodologia para identificação de pacientes portadores de aneurisma de aorta ascendente (AAAs) sob alto risco de remodelamento aórtico não está completamente definida. Objetivo Esta pesquisa objetiva caracterizar numericamente o fluxo sanguíneo aórtico, relacionando a distribuição do estresse mecânico resultante com o crescimento de AAAs. Métodos Estudo analítico, observacional, unicêntrico, em que um protocolo de fluidodinâmica computacional (CFD - Computacional Fluid Dynamics) foi aplicado a imagens de angiotomografia computadorizada (ATC) de aorta de pacientes portadores de AAAs. Duas ATC de aorta com pelo menos um ano de intervalo foram obtidas. Dados clínicos dos pacientes foram registrados e, a partir das imagens de ATC, foram gerados modelos tridimensionais. Foram realizados estudos do campo de velocidade e estruturas coerentes (vórtices) com o objetivo de relacioná-los ao crescimento ou não do aneurisma e, posteriormente, compará-los com os dados clínicos dos pacientes. O teste de Kolmogorov-Smirnov foi utilizado para avaliar a normalidade da amostra e o teste não-paramétrico Wilcoxon signed-rank foi aplicado para comparações de dados pareados entre os ângulos aórticos. A significância estatística foi fixada em 5%. Resultados Para o grupo que apresentou crescimento do aneurisma, a incidência do jato na parede aórtica gerou áreas de recirculação posterior ao jato, induzindo à formação de vórtices complexos, ocasionando um incremento na pressão média no endotélio aórtico. O grupo sem crescimento do aneurisma apresentou diminuição na pressão média. Conclusão Este estudo piloto mostrou que a CFD baseada em ATC pode, em um futuro próximo, ser uma ferramenta auxiliar na identificação dos padrões de fluxo associados ao processo de remodelamento de AAAs.

Abstract Background A methodology to identify patients with ascending aortic aneurysm (AsAA) under high risk for aortic growth is not completely defined Objetive This research seeks to numerically characterize the aortic blood flow by relating the resulting mechanical stress distribution with AsAA growth. Methods Analytical, observational, single-center study in which a computational fluid dynamics (CFD) protocol was applied to aortic computed tomography angiogram (CTA) images of patients with AsAA. Two CTA exams taken at a minimum interval of one year were obtained. From the CTA-gathered images, three-dimensional models were built, and clinical data were registered. Study of velocity field and coherent structures (vortices) was performed aiming to relate them to the presence or absence of aneurysm growth, as well as comparing them to the patients’ clinical data. The Kolmogorov-Smirnov test was used to evaluate the normality of the distribution, and the non-parametric Wilcoxon signed-rank test, for non-normal distribution, was used to compare the paired data of the aortic angles. Statistical significance was set at 5%. Results The incident jet in the aortic wall generated recirculation areas in the posterior region of the jet, inducing complex vortices formation in the group with aneurysm growth, leading to an average pressure increase in the ascending aortic wall between exams. In the group without aneurysm growth, the average pressure decreased. Conclusion This pilot study showed that CFD based on CTA may in the near future be a tool to help identify flow patterns associated with AsAA remodeling process.

Abstract Samanea tubulosa Benth. it has been widely used in traditional medicine to treat inflammatory processes. The present study aimed to investigate the antinociceptive effect and mechanism of action of the fractions obtained from the Samanea tubulosa pods in mice. The antinociceptive activity was evaluated in formalin, capsaicin and glutamate tests and the. The possible mechanisms of action involved in the antinociceptive effect of the hexane and ethyl acetate fraction in the opioid system, also the the K + ATP channels and the L-arigine pathways of nitric oxide were evaluated. The chemical characterization analysis revealed in the hexane fraction the presence of triterpenes such as lupenone and lupeol. In the glutamate test, the hexane and ethyl acetate fractions showed antinociceptive activity at the dose of 12.5 and 25 mg kg-1. The antinociception produced by the hexane and ethyl acetate fractions was significantly reversed by naloxone, indicating that the fractions act through the opioid pathway. Antinociceptive response of the ethyl acetate fraction was blocked by glibenclamide, indicating that this fraction acts via the K + ATP channels activation. It is concluded that the fractions under study exert antinociceptive activity possibly related to the opioid route and through K+ ATP channels activation.

Abstract Morphological characteristics of pequi fruits (Caryocar brasiliense) allows us to obtain information for the conservation, breeding, and detect genetic variability. Thus, the aims of this research were to characterize the genetic diversity of Caryocar brasiliense that produce fruits with and without thorn at the endocarp; determine the important characters in the study of genetic diversity, and to estimate phenotypic and genotypic correlations. 80 fruits with thorns and 90 fruits without thorns were evaluated. Data were used for analysis: Euclidian distance as a measure of dissimilarity and the Tocher method for delimitation of groups; Principal Component Analysis, and genotypic and phenotypic correlations. Genetic dissimilarity of C. brasiliense ranged from 0.066 (B6-D6) to 0.908 (A14-B3). Tocher method allowed dividing the matrices into three groups, the dendrogram into five groups, and the principal components allowed the distribution in 8 groups. We observed 15 significant correlations for characters of fruit with thorns and 31 correlations for characters of fruit without thorns. The weight of the fruit (with thorns) and the pyrene yield (without thorns) are relevant correlations for production. The groupings generated allowed the classification of the matrices into distinct sets. Allowed the identification of potential producers plants that could be used in breeding programs.
